    Sage Memorial Hospital to host second Affordable Care Act health fair

    Sage Memorial Hospital is scheduled to host their second Affordable Care Act (ACA)  health fair from 8:00 a.m. to 4:00 p.m. on Monday, Dec. 29th, in the Wellness Gym located on campus in  Ganado. The health fair is open to community members who need to enroll for health insurance, file a  health coverage exemption, or who qualify for Arizona Health Care Cost Containment System (AHCCCS) or Medicaid/Medicare benefits.

    The health fair is a joint and collaborative effort by Sage Memorial Hospital, and Indian Health Services  (IHS). This is part of a Navajo Nation-wide effort by the Navajo Department of Health and IHS.

    Community members who have not yet enrolled for health insurance or filed for exemption are strongly encouraged to attend.

    Individuals enrolling for health insurance or filing for health coverage exemption will need to bring the following documents:

     Certificate Degree of Indian Blood (CDIB), tribal identification or BIA Form

     IHS eligibility letter—if unable to provide, bring CDIB, ID or BIA Form

     Paystubs, W-2 or current Income

     Current employer health insurance

    Individuals applying for Arizona Health Care Cost Containment System (AHCCCS) or Medicaid will need to bring the following documents:

     Photo identification card

     United States Birth Certificate and/or Certificate Degree of Indian Blood

     Social Security Card

     Medicare Card, supplemental insurance cards, other health insurance cards if applicable

     Marriage certificate, divorce papers, or death certificates for spouses if applicable

     Veteran discharge document for applicant and/or spouse

     Copies of all Guardianship or Power of Attorney documents

     Residency verification showing county of residence

     Documentation of the sources of all income including Social Security, interest, and/or royalties

     Documentation of ALL financial resources

     Copies of prior income tax returns

    According to the Internal Revenue Service, provisions of the ACA regarding premium tax credits and individual shared responsibility will affect income taxes when 2014 tax returns are filed beginning next year. This includes possible payments, taxes or penalties being assessed on individual tax returns.
