Description:

Sage Memorial Hospital will like to introduce our October 2017 Employee of the Month, Mr. Brian Johnson. Mr. Johnson has worked in our Nursing Department as an Emergency Room Technician for the past 5 years. He is known for his sense of humor and incredible personality that both patients and staff find a delight. He is a pillar in his department for his leadership skills and can do attitude. He is always willing to go above and beyond for both staff and patients.

We would like to thank Mr. Johnson for all his hard work in helping provide quality healthcare to our patients here at Sage Memorial Hospital. On behalf of the Board of Director, Administration, and the Employee of the Month Committee we applaud you for your impeccable work.

Every month, Sage will choose an Employee of the Month based off of nominations by other employees. The EMPLOYEE OF THE MONTH will receive a gift card, a certificate and an award for the employee’s recognition.  A designated parking spot will also be included.  Selection will be announced on the first of each month by Meditech or E-mail.

Instructions for nomination:

Please submit your nomination through Meditech or Email to the attention of Anthony Castillo in the Human Resources Department.

Process:
1. Select any employee who you think deserves recognition for all the hard work and outstanding efforts he/she puts forth here at Sage Memorial.
2. Write a brief description or statement or a short success story indicating the reason this person should be selected as an employee of the month. Please indicate how this person has improved within your department. How he/she works with others and how he/she assists you and importantly with the patients.
3. Send your nomination requests by email or by Meditech no later than the 30th day of month.
