GANADO, Ariz. – Sage Memorial Hospital is proud to offer a variety of services through our mobile health unit for those residing in the rural areas around Sage Memorial Hospital.
Please see attached for the August schedule of the Mobile Health Unit (for distribution). The Sage Memorial Hospital Mobile Health Unit provides the following services, subject to varying days and locations:
- All-inclusive Diabetes Care
- Management of Chronic Health Conditions
- Physicals for School / Employment (12 years and older)
- Immunizations for Adolescents & Adults
- Quick STI testing & treatment
- Prescription refills
- Referrals
Services are for individuals 12 years and older, and a physical form needs to be filled out and signed. A parent or guardian must accompany minors during their examination.
