Sage Memorial Hospital Community Health Department Announces July 2024 Schedule of Services

The Community Health Department is pleased to present its schedule of health services for July 2024.

Key Highlights for July 2024:

  • Community Health Nursing Home Visits: Our dedicated Community Health Nurses (CHN) will conduct home visits on July 1, 5, 8, 12, 15, 19, 22, and 29.
  • Infectious Disease & Diabetes Clinic: Specialized clinics for Infectious Disease and Diabetes management will be held on July 3 and 31. These clinics provide crucial screenings and education for individuals affected by these conditions.
  • Mobile Health Unit Services: Our Mobile Health Program (MHP) is scheduled to visit multiple locations throughout the month, offering services such as diabetes screening and education, sexual healthcare, immunizations, harm reduction services, and gender expansive care. Walk-in services, medication refills, and sports physicals are also available.
    Key dates include:

    • Cornfields Chapter: July 18 from 10AM to 2PM
    • Nazlini Chapter: July 23 from 10AM to 2PM
    • Burnsides Lowe’s: July 26 from 10AM to 2PM
    • Steamboat Chapter: July 30 from 10AM to 2PM
  • Produce Pilot Prescription Program (P4) Cooking Classes: The P4 program will conduct cooking classes to promote healthy eating habits. These classes will be held at various chapters from 10 AM to 2 PM:
    • Klagetoh Chapter: July 16
    • Kinlichee Chapter: July 17
    • Greasewood Chapter: July 24
    • Wide Ruins Chapter: July 25
  • Sports Physicals Event: A special event for sports physicals will be held at the Ganado Flea Market on July 27 from 10 AM to 2 PM.
