Please find attached the Mobile Health Unit NOVEMBER 2024 Schedule (for distribution). Services provided by the Sage Memorial Hospital Mobile Health Unit (available on varying days and locations) include:

Please note that Mobile Health Unit’s operation days and times are subject to change. Services are for individuals 12 years and older. A physical form MUST be completed and signed. 2nd – Infectious Disease & Diabetes Clinic 8am – 5pm

3rd – Mobile Clinic, Nazlini 10am – 2pm

Keep your health in-check with CHN Home Visits on 4th, 5th – 7th.

8th – Continue your healthcare journey at Mobile Clinic, Klagetoh (10am – 2pm)

9th – Infectious Disease & Diabetes Clinic 8am – 5pm

10th – Greasewood Clinic 9am – 3pm

Once again, our CHN Home Visits available on 11th, 12th – 14th.

15th – Mobile Clinic, Cornfields 10am – 2pm

16th – Infectious Disease & Diabetes Clinic 8am – 5pm

Stay protected with us on 17th, 18th & 19th with Mass Flu Vaccination (Student Day) and Mass Flu/COVID Vaccination (Community) from 8am – 5pm

20th – 21st CHN Home Visit to keep your family safe.

22nd – Mobile Clinic, Greasewood Chapter 10am – 2pm

23rd – Infectious Disease & Diabetes Clinic 8am – 5pm

24th – Greasewood Clinic 9am – 3pm
