September 2023 Schedule: Sage Memorial Hospital’s Mobile Health Unit

GANADO, Ariz. – Sage Memorial Hospital’s mobile health unit is dedicated to providing a range of outpatient services to our rural communities surrounding our service area.

Please find attached the Mobile Health Unit September 2023 Schedule (for distribution) Services provided by the Sage Memorial Hospital Mobile Health Unit (available on varying days and locations) include:

DIABETES SERVICES:

  • Comprehensive DM Care
  • Medication Refills
  • Adult Immunizations

OTHER SERVICES:

  • Chronic Health Management
  • School/Employment Physicals
  • Express STI Testing and Treatment
  • Referrals

Please note that Mobile Health Unit’s operation days and times are subject to changes. Services are for individuals 12 yrs. and older. Physical form MUST be completed and signed. Sage Memorial Hospital Mobile Health Unit schedule for August 2023

GANADO, Ariz. – Sage Memorial Hospital is proud to offer a variety of services through our mobile health unit for those residing in the rural areas around Sage Memorial Hospital.

Please see attached for the August schedule of the Mobile Health Unit (for distribution). The Sage Memorial Hospital Mobile Health Unit provides the following services, subject to varying days and locations:

  • All-inclusive Diabetes Care
  • Management of Chronic Health Conditions
  • Physicals for School / Employment (12 years and older)
  • Immunizations for Adolescents & Adults
  • Quick STI testing & treatment
  • Prescription refills
  • Referrals

Services are for individuals 12 years and older, and a physical form needs to be filled out and signed. A parent or guardian must accompany minors during their examination. Sage Memorial Hospital Cafeteria Opens its Doors to the Public

GANADO, Ariz. – Sage Memorial Hospital (SMH) is excited to announce the opening of their cafeteria to the public. After a successful month of trial runs, SMH is now welcoming local community members and visitors alike to enjoy their delicious meals in a safe environment. The hospital had previously limited access to its cafeteria to staff and patients.

“We are excited to open our cafeteria doors to the general public,” said Nathan Tsosie cafeteria supervisor at SMH. “We have taken the necessary safety precautions to ensure that our cafeteria is a safe and enjoyable place for everyone.”

Safety measures in place include social distancing markers, mask-wearing requirements, additional hand sanitizer stations, and frequent cleaning of frequently touched surfaces. Cafeteria seating has also been rearranged to allow for better physical distancing between customers.

The cafeteria is open Monday – Friday from 6:00am to 5:30pm with last orders at 5:00pm. The cafeteria offers a wide range of delicious healthy meals, snacks, beverages and daily specials. The menu includes breakfast and lunch items such as egg dishes, sandwiches, salads, burgers and more. Weekly menus and online ordering is available at www.toasttab.com/sagecafeteria.
